 From the spec definition of itemResult.identifier...

"The identifier of the item for which this is a result. For item results

that are reported as part of a test result this is the identifier used 
to refer to the item in the test (see assessmentItemRef). For item 
results that are reported on their own, this can be any suitable 
identifier for the item. Where possible, the value should match the 
identifier  attribute on the associated assessmentItem."

So in short, it will be the identifier you used on the
assessmentItemRef.

Although item identifiers might be reused in assessmentItemRefs it is 
also quite common for people to use a completely different scheme.  For 
example, when items are drawn from a bank and placed in a test the 
assessmentItemRefs might have an identifier scheme like: Q1, Q2, Q3,... 
etc.  But, the items being referred to won't normally have matching 
identifiers as they're more likely to have 'unique' ids assigned by the 
bank or the original authoring system (cf. the "Message-ID" header in 
email messages).
